Case presentation

A 78-year-old man is admitted to hospital via the emergency department following a collapse. He is suspected to have sustained head trauma during the incident. PMH. He suffers from hypertension, biventricular heart failure, chronic kidney disease and osteoarthritis of both hips. Serum biochemistry on arrival shows Na+ 130 mmol/L (137-144), K+ 6.2 mmol/L (3.5-4.9), U 8.5 mmol/L (2.5-7.0) and Cr 128 µmol/L (60-110). His current regular medicines are listed opposite.
